The American with gender neutral passport is going somewhere, i presume. The TSA has no jurisdiction elsewhere. So when the American lands in Brazil the customs officials can do whatever they want including stripseach.

Passengers are required to be patted down by an agent of the same gender. I'd guess an easy 99% of the time, agents can tell someone's gender just by looking.
So I guess for that rare 1 percent of the time, listing gender on passports would actually be useful.

Either way, Duchess brought up pat-downs, which would have rare instances where someone's gender needs to be confirmed.
