An interesting thing about "the U.S.A." is that, unlike nearly all other countries around the world, its official demonym is at the root of the confusion. No "Columbians" are going to say "Hey, don't call me a Columbian, I'm an American in Columbia", not in a regular conversation, anyway.
It didn't stop, it's just in our minds that America is USA.

And yet their demonym is "Colombian"? Have some Colombians been telling you their national demonym is "American"?

A Colombian is a Colombian and a South American, historical rhetoric about The Americas and The New World notwithstanding.
Like I introduce myself as being Irish, but I'm still European. Same difference.
