You use it to imply that anyone who does not care for homosexuality, fears it. You use it to call someone a name.

I got my definition from dictionary.com. Merriam-Webster agrees (irrational fear of, aversion to, or discrimination against homosexuality or homosexuals).

I would guess at one point it was strictly fear of but has now broadened to become the meaning above.
