Speaking as a pretty religious Sicilian player, I have personally never lost to the Smith-morra gambit. I'm not sure if it's because of the gambit's unsoundness or my opponents' weak play, but I'm pretty sure it's unsound, as my defensive technique is pretty lacking. I find the most annoying anti-sicilian to be the Grand Prix Attack.
The Smith Morra essentially gives away a pawn for an attack. White naturally has a slight plus to begin with, so why risk it? A bird in the hand..., as they say. You can still get an attack with a half-dozen lines, like the Grand Prix, the English Attack setup, etc. The SM is pretty well studied into the endgame, when, if careful, black can win with that pawn.
