wasnt there that insane opening called the damiano defence 1e4 e5 2Nf3 f6 3Nxe5!! fxe5 4Qh5+ and black is really struggling, surely the same will happen if you play Nf3 in response to f6 therefore you cant play e5 afterwards?

He mentioned in response to 1.d4, not e4. This has in fact, been tried in practice.

I think 9. Qg5 might be an improvement for white. The position is still unclear, though.

If you don't like the Caro-Kann, don't play it. But please don't insult the players here that know something about the Caro-Kann.

In the variation cited, Black isn't just "struggling": he's almost certainly dead lost. White will get his material back with interest, and Black will have a wretched position. Relatively best for Black is 3...Qe7, in which case he's definitely "struggling", but not totally lost.
