How far back do you have to go to find one exception?
Face it. Only imperialist presidents get second terms since the Spanish American war except those that were assassinated. McKinley would have gotten a second term if he had not been assassinated for being an imperialist. He was a puppet of the Rockefellers and JP Morgan.

I have noticed that presidents in recent history only get a second term if they invade other countries or escalate wars that have already been started by a previous presidents.
Are there any exceptions? If so, how many?

But with there having been only about a dozen presidents in "recent history," the sample size is so small that I doubt anything significant could be gleaned from whatever pattern you think you discern. There are just too many variables.
In a way, this thread is something of a textbook insight into the workings of the conspiratorial mind, which seeks patterns in everything right down to the grill marks on a hamburger.

He did not follow through with turning Iraq into a puppet state of the USA. His son did though and got a second term.

Johnson's only chance at becoming POTUS was the assassination of JFK. Johnson was a huge creep. He was even very childish at times. He would push people like a schoolyard bully. Johnson’s influence was also limited as vice president, as Kennedy’s advisers (especially his brother and attorney general Robert Kennedy) made sure to keep him on the sidelines. During the Cuban Missile Crisis, for example, Johnson was a member of the group convened to advise the president, but was excluded from the meeting at which the final decision about the American response was made.
Vietnam was also a huge failure for the USA. The first war the USA ever lost.
