My wife has worked for various aid agencies for the past 10 years or so. A large proportion of the money involved goes back to the source country (sometimes it is a condition of the aid - eg British aid agencies have traditionally insisted that their expatriates only drive landrovers).
My sister is a farmer and she assures me that if the US was to stop farm subsidies it would benefit Africa more than total world aid. In fact the US does spend more on farm subsidies than total world aid.
Some types of aid are very damaging. Food relief for example, in Zambia, has regularly damaged the economy - especially the farmers.
Some aid however has been beneficial. For example aid agencies have dug a lot of boreholes which provide clean water for many Zambian villages.
