What happened there?

He was on a film set and fired a gun obviously thinking the gun was loaded with blanks. However it had been used for target practice by the film crew and contained live rounds. The cinematographer was killed and another crew member injured. He is being charged with unlawful killing.

How is it possible to hold him responsible for something that should have been checked by somebody else?

The person who should have checked it is also being charged. Its like in a company. If someone gets killed in an industrial accident the directors or owners can be charged with manslaughter. At least thats how it works in the UK.
Alec Baldwin was also the producer.

The accident came about through a concatenation of stupidities. Moreover, the script did not call for the actor to pull the trigger, but only to point the weapon at the camera.

I read the film will be released. Betcha that scene will be edited out,
